Who is Kenjaku in JJK?
Kenjaku is the ancient sorcerer behind almost every major disaster in Jujutsu Kaisen. He survives by moving his brain from body to body, and for most of the series he's wearing Suguru Geto's. If you've wondered why "Geto" has stitches across his forehead in Season 1, Kenjaku is the answer.
The "kenjaku jjk" question most people start with is simply: who is this, and why is he wearing Geto? The short answer is above. The long answer runs through a thousand years of history.
Personality
Kenjaku is polite, curious and utterly without conscience. He treats the world like a long experiment and other people, sorcerers, curses and humans alike, as materials. He's patient in a way no other villain in the series is. Plans that take centuries don't bother him. He's also genuinely interested in what humans might become, which is the closest thing he has to a motive.
Appearance
Because he changes bodies, Kenjaku doesn't have one appearance. The constant is a line of stitches across the forehead, where the skull has been opened for his brain. When the top of the head opens, you see the brain itself, with a mouth. In the main story he looks like Geto: long black hair, monk's robes, the same calm face.
Background
Kenjaku has been alive, in one body or another, for roughly a thousand years, since around Sukuna's time. He has worked with Sukuna, plotted against Tengen and manipulated jujutsu society from inside for generations.

In the Meiji era, around 150 years before the story, Kenjaku was Noritoshi Kamo, remembered as the worst sorcerer in history. In that body he created the nine Death Paintings, Choso and his brothers among them. Later he took the body of Kaori Itadori, Yuji's mother, and gave birth to Yuji, designing him as a vessel for Sukuna. After Geto's death at the end of JJK 0, he took Geto's body to get his Cursed Spirit Manipulation and, crucially, Geto's face, which he knew would stop Gojo in his tracks.
Cursed techniques
Kenjaku's innate technique is the body swap itself: by transplanting his brain, he takes over a body and can use the technique that came with it. In Geto's body, that means Cursed Spirit Manipulation, absorbing and commanding cursed spirits, including the ability to use their techniques in some cases. He's also extremely knowledgeable about barriers and binding vows.

From Kaori's body, Kenjaku also has access to Antigravity System, which he uses later in the story. His domain expansion is Womb Profusion. He also absorbs Mahito in Chapter 133 and uses Idle Transfiguration's power to trigger the awakening of cursed techniques across Japan.
Kenjaku's role, arc by arc
Season 1: as "Geto", he works in the background with Mahito, Jogo and Hanami, and plans the theft of Sukuna's fingers and the Death Paintings during the Goodwill Event.

Shibuya: he springs the trap on Gojo and seals him in the Prison Realm in Chapter 91. At the end of the arc he absorbs Mahito and announces the Culling Game.
Culling Game: Kenjaku's master plan is to use the game to merge humanity with Tengen. He kills Yuki Tsukumo in Chapter 208.
Shinjuku: After Gojo's death, Kenjaku is fighting several sorcerers at once when Yuta kills him in Chapter 243. Rika consumes him, which lets Yuta copy his body-swap technique later.
Kenjaku's master plan
For most of the story, Kenjaku's goal is a mystery. The Culling Game explains it. He wants to merge humanity with Tengen, using the cursed energy created by the colonies to trigger a forced evolution of the entire population. Everything else, from creating the Death Paintings to designing Yuji as a vessel to sealing Gojo, is in service of that.
That makes him a very different villain from Sukuna. Sukuna wants to enjoy himself. Kenjaku wants to see what humanity could become, and he's willing to kill millions to find out. The sheer scale of his patience is what makes him frightening.
Kenjaku and Gojo
Kenjaku knows Gojo is the only person who can wreck his plans, so he builds them around neutralising him. Using Geto's body is part of that. He knows that Geto's face will make Gojo hesitate, even for a moment, and in Shibuya that moment is all he needs.
Tracking Kenjaku through the story
Kenjaku is hidden in plain sight for much of the series. Rewatching with the twist in mind is a strange experience, because the clues are all there.
The end of JJK 0. A figure with Geto's face and a stitched forehead appears after Geto's death. On release it was a cliffhanger. Now it's the first look at Kenjaku in his current body.
Season 1 and the Death Paintings. "Geto" works with Mahito and the Disaster Curses, giving orders in a voice that sounds just like the real one. Watch his interest in Yuji. It isn't casual.
Shibuya. He seals Gojo using the Prison Realm, then reveals himself, removing the top of his skull. He also hints at earlier identities, including Noritoshi Kamo, who created the Death Paintings.
The Culling Game. Kenjaku sets up colonies across Japan and gives the players their rules. Every arc after this is chasing his plan.
For a quicker catch-up, the Shibuya reveal episode and the first Culling Game chapters cover most of what you need.
Relationships
Sukuna is an ally of convenience, tied by a binding vow. Uraume works with him for Sukuna's sake. Mahito is a partner he eventually consumes. Choso and Yuji are, in different senses, his children, and both hate him. Gojo and Tengen are the obstacles he's built his plans around.
